New trichology clients, please complete the hair-growth questionnaire before your scalp analysis. If you're coming in for a scalp condition (eczema, psoriasis, dermatitis), the intake form saves us twenty minutes of your appointment.
Alma TED™ appointments follow an initial consultation and require a signed consent form; TED bookings are reserved with an appointment fee.
